These tendencies to wrap us is what many people call "denial", which creates a false perception of self and inability to accept the truth about us. As such it becomes painful to accept mistakes leading to failure in accepting responsibilities.
Lack Of Planning
A wise man once said that failing to plan is planning to fail. Planning is a necessary skill in life if success is to be achieved by any individual or organization.

Laziness can be defined not willing to work or be energetic. It can also be defined as conducive to indolence or inactivity.
A person is being lazy if he or she is being able to carry out an activity but refuses to do so because of the effort involved. This is an enemy of success and if not conquered can lead to failure in life.
Learning From The Mistakes Of Others
They say we learn from our mistakes but it has been discovered that we can be more successful and even get more when we learn from the failures of others.
A study conducted at Bristol University led by the Faculty's Dr Paul Howard-Jones and Dr Rafal Bogacz, used brain imaging to look at how people and animals learn from failures and success.
The researchers scanned the brain of human that battled against an artificial opponent in a computer game. The scanned brain showed that while players learned from their own successes, they had very little reaction to the opponents win but when the computer failed the task, the brain actively jumped back up. When opponents messed up, players experienced responses in both rewards and learning signals in the brain, which suggests that we take notice of our competitors failures and also learn from the action that caused it, to avoid making similar mistakes in the future.
Lack Of Clear Goals
Setting clear goals gives your life direction, boost your motivation and self confidence. Whoever wants to succeed in life must learn to set clear and achievable goals. Goal setting not only allows you to take control of your life's direction, it also provides you a benchmark for determining whether you are actually succeeding.

that start with careful consideration of what you want to achieve and ends with a lot of hard work to actually achieve it.
